THE APPLICATION PROCESS
How do I apply?
>Visit the admission section on our website. Download forms. Call if you have any questions.
>You are also welcome to make an appointment to meet with the program director and take a tour of the center.
Are you able to care for individuals...
>Who are incontinent? Yes. We are one of the day programs in the Northern Virginia area who accept individuals with incontinece.
>With difficult behaviors or who are aggressive? In most cases, Yes. It is our philosophy that with the proper environment, activities and supervision, the difficult behaviors seen in individuals with Alzheimer's/Dementia can be managed.
>Who wander? Yes. Our facility has been customized for individuals with Alzheimer's/Dementia. We have over 6,000 square feet as well as two outdoor courtyards with a walking path. The facility is secure, providing participants a safe place to wander .
>Who do not have Alzheimer's/Dementia? Yes. Our facility also serves individuals with other related disablities. Contact us if you are unsure whether Hearth & Home would be right for your family member.
How long does the admission process take?
>Once the admission paperwork is complete, contact Hearth & Home's program director or healthcare coordinator to set up an intake meeting. Upon completion of the intake, the participant can be admitted for services as soon as the next business day.
PROGRAM FEES AND OVERALL COST
How much will it cost for attendance?
>Private pay rates: 1-3 days/week $70.00 per day, 4-5 days/week $60.00 per day, Half day up to 5 hours a day $50.00. Enhanced Fee: $15.00 added to base fee (see Caregiver Handbook for details).
Is adult day healthcare covered by Long Term Care Insurance?
>Most long term care insurance polices often pay the entire cost of full time attendance. Check with your insurance agent to be certain you covered under the policy. Often there is a co-pay or specific amount to be paid out of pocket before the LTC insurance will be paying.
Do you accept Medicare/Medicaid?
>No. Medicare will not cover adult day care services.
>Yes. Medicaid will pay some or all of the daily rate if you are a Medicaid recipient.
Do you accept VA benefits?
>No. Hearth & Home ADHC is not contracted to provide services to VA recipients.
Will health insurance cover the cost of your program?
>Most health insurance programs will NOT cover adult day care services.
HOURS AND SERVICES
What are your hours of operation?
>Hearth & Home is open Monday through Friday from 6:30am to 6:00pm.
Do you provide care on the weekends?
>Currently this is not a service we provide.
What services does the center offer?
>Nursing supervision and care, including medication administration, health monitoring, and response to medical emergencies
>Assistance with personal care needs such as: walking, transferring, eating, toileting, personal hygiene.
>Meals offered: continental breakfast, mid-morning snack, catered lunch and afternoon snack. We are able to accomodate the following diets: regular, no added salt and diabetic alternatives.
>Comprehensive daily activity program based on individual's social, emotional, cognitive and physical needs.
>Salon services provided by a licensed beautician.
>Podiatry services are offered every 8 weeks.
>Rehabilitation Services (Physical Therapy, Occupational Therapy, Speech Therapy).
What do your participants do all day?
>Scheduled daily activities may include: physical and mental exercises, music and dancing, arts and crafts, intergenerational programs, table board games, indoor sports, bingo, montessori based programming, movies, spiritual involvement, birthday parties, ice cream socials, cards and walking club. Volunteers come to provide pet therapy, piano playing, puppet shows and bingo.
TRANSPORTATION
Do you provide tranportation to the center?
>Transportation is NOT a service we currently provide.
OTHER QUESTIONS
Are you a licensed adult day care center/program?
>Yes. The center is licensed by the Department of Social Services and Medicaid.
Are there volunteer opportunities at the center?
>Yes. We value all volunteers as they enhance our program in many ways. Volunteers must be at least 13 years of age or older. To learn more about our volunteer opportunities, please contact the program director or nurse.
Are you affliated with any other organization?
>Yes. We are a ministry of Manassas Baptist Church, a not-for-profit 501(c)(3) organization with a board of directors.
>Yes. We are members of the Virginia Adult Day Services Association (VADSA).
