The Virginia Department of Social Services licensing division requires the following forms to be completed by a physician within thirty (30) days prior to admission: Medical Examination Report (includes TB Screening) and Medication List. The following must be completed by a family member: Activities of Daily Living (ADL's) Assessment, Activity Interest Survey and the Authorization for Participant Pick-Up forms. When the above paperwork is completed, return all forms to the center for processing. Upon review by the Program Director and Healthcare Coordinator an intake meeing will be scheduled with you by the Healthcare Coordinaotor. Intake appointments are available Monday thru Friday between 10am and 2pm and 2:30-4:30pm. Please allow at least an hour to complete the forms for enrollment. Once the intake process is completed then admission will follow as soon as the next business day.

Please bring any legal documents (i.e. advance directives, power of attorney, etc.) as well as insurance cards. We will need to make copies for the participant chart.
